Abstract:
Due to profound changes occurring in biomedical knowledge and in health systems worldwide, an entirely new health and social care scenario is emerging. Moreover, the enormous technological potential developed over the last years is increasingly influencing life sciences and driving changes toward personalized medicine and value-based healthcare.

However, the current slow progression of adoption, limiting generation of healthcare efficiencies through technological innovation, can be realistically overcome by fostering convergence between a Systems Medicine approach and the principles governing Integrated Care. Implicit with this strategy is the multidisciplinary active collaboration of all stakeholders involved in the change.

The Catalan open innovation hub on ICT-supported integrated care services for chronic patients aims to consolidate a collaborative network promoting regional deployment of enhanced care for chronic patients encompassing vertical integration, between specialized and primary care, as well as horizontal integration among community-based healthcare and social services.

Main goals are: 
(i) Promotion of ICT-supported value-generating services with a preventive focus; (ii) To develop & apply innovative evaluation methods in real world settings; (iii) Develop multilevel subject-specific risk predictive modelling feeding clinical decision support systems / patient decision support systems (CDSS/PDSS); (iv) Develop & implement cloud-based computing environments; and, (v) Refine application of the regulatory frame for evaluation in real-world settings.
